CONFERENCE REGISTRATION:
Early bird registration forms before April 15, 2014 is $20.00
Registration forms between April 16 to April 30, 2014 is $30.00
Wisdom Steps Participants will receive a certificate, pin and/or charm, tote, t-shirt, Wednesday breakfast, awards dinner and
Thursday morning breakfast.
Non-Wisdom Steps participants by April 30, 2014 is $100.00.

Wisdom Steps participants must register with your local Wisdom Steps coordinator. To be eligible to attend the conference, participants must have completed and turned in their STEPS to their local contact person by December 31st 2013. Also the completed health record must be turned into their local contact person.
